Glow Up Aesthetics Policies and Patient Agreement

Our Promise to Patients

Glow Up Aesthetics P.C, strives to provide quality care to each patient at each visit. We prefer a customized approach based on an individual’s goals and baseline conditions/history. Treatment plans are best devised when both patient and provider are in agreement – with a focus on safety, realistic expectations & timelines, as well as desired & reasonable outcomes. Out of convenience and courtesy to patients, we offer one free treatment consult per calendar year to facilitate thorough assessment and to formulate a treatment plan.  

 

Refunds | Exchanges

Due to the variables involved in aesthetic, cosmetic, and dermatologic procedures, results and outcomes are not guaranteed. All services and procedures are non-refundable and non-transferrable. You, as the patient, assume the responsibility of fully reading each individual procedure’s consent form and acknowledge your opportunity to ask any questions of your provider prior to each procedure. For all retail products, including medical grade skin care products, exchanges and refunds will not be given unless the product is found to be defective at the time of purchase.  

 

No-Show | Cancellation Policy

We do our best to see patients in a timely manner. We ask that you also make every effort to arrive on time. If you are more than 10 minutes late for a scheduled appointment, we may not be able to honor your appointment and you may be asked to reschedule. Should you need to cancel or reschedule, we require more than 24 hours notice. We understand emergencies happen but the courtesy of calling is all we ask. A no-show will result in a $50 fee. We reserve the right to discontinue treatment/ discharge patients from the practice for habitual no-shows.

 

Electronic Communication

By texting or sending pictures/videos to our phone (269-692-5044 Kalamazoo, 616-433-2882 Grand Rapids) or by messaging our social media accounts, the patient assumes the risk of possible health information compromise, as they are not HIPAA protected means of communication. 

 

Acknowledgment and Receipt of Privacy Policy

Glow Up Aesthetics, P.C. is required by US Federal Law and the Health Insurance Portability & Accountability Act of 1996 (HIPAA) to maintain our patients’ privacy and provide them with access to the notice of our legal duties. 

You have the following rights to your protected health information, (PHI) which you can exercise in a written request for the following: 

  • The right to request restrictions on certain uses and disclosures of PHI to family members, friends or anyone else identified by you. We are, however, not required to agree to a requested restriction. If we agree we must abide by it until the restriction is removed by you in writing.
  • The right to reasonable requests to receive confidential communications of PHI from us by alternative means or at alternative locations.
  • The right to inspect and copy your PHI.
  • The right to amend your PHI.
  • The right to receive an accounting of disclosures of PHI.
  • The right to obtain a paper copy of this notice from us upon request.
